The JaxHATS clinic is a "Medical
Home" for youth, ages 16-26 with chronic medical or developmental problems
in the counties of Duval, Nassau, Baker, Clay, and St. Johns in Florida. JaxHATS
has a multi-disciplinary staff that includes a pediatrician, adult internal
medicine specialist, nurse care coordinators and a transition specialist who
provide primary medical care to address the young person's immediate medical
needs. The JaxHATS team also works with youth over time, to help them gain the
knowledge and skills they need to achieve their long term life goals. Services
provided by JaxHATS are covered by Medicaid, Children's Medical Services and
most other health insurance plans.

supports the shift of responsibilities from parents to young adults;
prepares young adults to be active members of the health care team;
and
promotes the development of self care skills and independence
